Please complete the information below in order to register for a Point and Click account. This is where you will submit all required health forms. The name, email, and phone number entered must exactly match what Culver has on file.

· Mobile Phone # (by entering your mobile phone below you opt in to receiving text messages for the purpose of confirming your Point and Click account)

· Parent/Guardian Date of Birth (the parent’s date of birth is used for account security purposes only, and you will need to confirm this date each time you log in)
